Cut a cardboard 11 3/8 x 8 1/4. 

Make a mark with a pen at 1 3/8, 4 1/8, 6 7/8 and 9 5/8

Fold at 2 3/4, 5 1/2, 8 1/4 and 11

Turn your paper and fold at 1 3/4.

Now for each panel fold as on the template:
